London authorities have released Ozzy Osbourne’s death certificate, confirming that the Black Sabbath frontman died of out-of-hospital cardiac arrest brought on by an acute myocardial infarction. The document, filed at a register office in Hillingdon and submitted by his daughter Aimée Osbourne, lists coronary artery disease and Parkinson’s disease with autonomic dysfunction as contributing factors. Osbourne’s occupation is recorded as “Songwriter, Performer and Rock Legend.” Osbourne died at his Buckinghamshire home on 22 July at the age of 76, according to the certificate. The disclosure ends two weeks of speculation about the cause of death of the self-styled “Prince of Darkness,” whose family had initially withheld medical details. The certificate’s release follows a large public farewell in Birmingham, where thousands of fans lined the route of a funeral cortege that paused at the Black Sabbath bench and bridge before a private burial on 31 July. Osbourne’s final live performance came just 17 days before his death, when he reunited with Tony Iommi, Geezer Butler and Bill Ward at Villa Park for the “Back to the Beginning” concert. A pioneer of heavy metal, John Michael “Ozzy” Osbourne sold more than 115 million records with Black Sabbath and as a solo artist during a six-decade career that also encompassed a Grammy-winning 2022 album and a reality-television franchise. He had spoken openly about his Parkinson’s diagnosis in 2020 and enduring mobility problems after a 2019 fall, but continued to express a determination to keep performing until shortly before his death.
