The U.S. Department of Justice (DOJ) under the Trump administration has initiated an investigation targeting gender-affirming medical care for transgender minors. The DOJ issued subpoenas demanding sensitive patient information from hospitals, including billing documents, communications with drug manufacturers, Social Security numbers, dates of birth, and addresses. One notable subpoena was directed at the Children’s Hospital of Philadelphia, seeking extensive records related to gender-related care. A federal judge has ordered the DOJ to clarify the scope of this investigation, which covers the provision of gender-affirming care to individuals under 19 years old nationwide, including in states where such care is legal. The DOJ has dismissed concerns about the potential trauma to patients as "meritless." This investigation and federal pressure have led several major hospitals and health systems, including Stanford, Kaiser, and the University of Michigan hospital system, to halt or restrict gender-affirming treatments and surgeries for minors. At least 21 hospitals have curtailed such care since January 2025, primarily in states with Democratic leadership. The University of Michigan announced it will cease providing gender-transition interventions to patients under 19 in response to threats of criminal prosecution from the DOJ. These developments reflect an aggressive federal strategy aligned with White House priorities aimed at limiting access to gender-affirming care for transgender youth. Additionally, the Department of Education has launched an investigation into a Massachusetts school district over a student survey, indicating broader federal scrutiny related to transgender issues.
